In the ‘before times,’ as people are now calling the period before COVID-19, dentists were already managing a highly demanding profession with significant mental health stressors. In fact, dentists have one of the highest rates of depression and suicide amongst health workers.

Now, with COVID-19, dentists are largely forgotten in the panoply of health providers working during the pandemic. They’ve been forgotten as both business owners with a high overhead to maintain their practices and safeguard their staff, and as front-line workers, who do essential and potentially life-endangering work requiring personal protective equipment (PPE).

This has meant the stress of dentists is at an all time high.

Replacing previous dentistry poses many challenges. This case serves as a good example. There are the problems that we see. The shade and the contours can be improved. Then there are the problems that we don’t see until the previous dentistry is removed. In this case, the buccal crown margin was deeply subgingival and open, which was contributing to the inflammation. ⁣
⁣
The solution is always a compromise between what is ideal and what our patients are willing to do in terms of treatment. The compromise may not be perfect but can often create a situation where the patient is happy and we have improved the clinical condition as well! 😊⁣

Multiple class II restorations on adjacent teeth are always a challenge. I am in awe of those practitioners who can prepare both teeth simultaneously and finish with great anatomy and contacts. ⁣
⁣
I need to prepare one tooth at a time in order not to lose the marginal ridge height of the neighbouring tooth as a guide.⁣
⁣
In this example, I restored tooth 24 DO first using 25 as my reference and then prepared 25 MOD using 24/26 to determine my marginal ridge heights. ⁣
⁣
I find that this method reduces the amount of adjustment that is needed when verifying the occlusion. ⁣
⁣

$✨Compliance is a Critical Factor to Success⁣✨⁣ ⁣⁣ Compliance of our patients is one of the most important determinants of success. This case is a perfect example of excellent compliance. The orthodontics challenges at the beginning of treatment are numerous. Our goals were to alleviate the crowding, expand the constricted arches, lessen the overbite and to create equal spaces between the teeth upper lateral incisors for future porcelain restorations. ⁣⁣ ⁣⁣ The success in achieving these goals with Invisalign is due to our patients compliance in wearing the aligners diligently for the full treatment time. Once the the teeth were in position, we were able to restore the the peg lateral teeth with conservative porcelain restorations and improve the embrasures between the canines and laterals with direct composite bonding. ⁣ ⁣ The Zeiss Extaro 300 microscope was used in the preparation of the teeth to produce a conservative but imperceptible margin and in the bonding of the canine teeth. Equilibrating the occlusion to centric relation, as taught by @dawsonacademy will ensure stability of the post orthodontic bite as well as providing the correct anterior guidance to prevent fracturing the restorations. ⁣⁣ ⁣⁣ Patient compliance is critical to so many of the treatments that we provide. I always approach switching to a new material with caution. This product however has the reputation of its former self to fall back on. I have been using Voco Grandio for my posterior restorations for years. The restorations are strong and wear quite well. Today, we tried Voco Grandioso, the newer version. The handling is different as the material is more firm. This lends itself quite well to modeling for an anatomical build-up. It also polishes quite effortlessly, which was not the case previous version. Thank you to @that_voco_guy_marion for the opportunity to try Grandioso!

The previous crown on the right lateral incisor had served the patient for many years. Unfortunately, it had cracked on the palatal aspect and required replacement. We took advantage of this opportunity to improve the colour match, add characterization and improve the gingival health at the crown margin. The preparation of the margin and a well contoured provisional were critical to improving the tissue health. Bonding to the right central and left lateral was also done to improve the aesthetics. Bonding to the canine teeth was done to provide disclusion in function. In addition, a nightguard is essential for this patient! ⁣

It was wonderful to have Rod and Lubna from Zeiss Canada at the clinic! I had the opportunity to talk about my favorite technology in the office, which is of course my microscope. The microscope has allowed me to practice ergonomically. It also helps me to see the treatment area in detail which allows me to reach my treatment goals when delivering care.

Our patient was concerned about her discoloured front tooth. The discolouration is a result of trauma to the tooth when she was younger. Composite resin was added to the tooth to repair a chip from the trauma. Additional composite was added to help with the discolouration. It was time to replace the composite as it was worn and discoloured. Shade matching a dark tooth is a challenge because we are limited to the degree that we can change the shade based on the thickness of the material. The more thickness of material that we need, the more tooth structure needs to be removed. In this case, only the pre-existing composite was removed. Ideally, a porcelain restoration will give better aesthetics and longevity. Until then, our patient is thrilled with the improvement in the look of her front tooth with composite! ⁣

August is coming to an end and at Soho Dental, we have had another exciting month doing what we love to do!⁣

Once again, a sincere thank you for everyone’s compliance to our safety protocols when coming in for your dental visit. Masks, social distancing and hand hygiene are the cornerstones to keeping everyone safe. ⁣

Aside from the visible safety measures that we have established, I wanted to bring awareness to some additional ones that are less obvious.⁣
⁣
Every treatment room has a medical grade HEPA filtration unit to circulate the air and filter out contaminants multiple times per hour. These are running all the time during clinic hours. ⁣
⁣
All treatment rooms, common areas and furniture get fogged daily with a Health Canada approved disinfectant that sanitizes difficult to access surfaces. ⁣
⁣
These are just additional measures that may have benefit. Ultimately, the most important safety measures that we can all carry out during our daily activities in all environments are to wear a mask in indoor spaces, to maintain social distance and to sanitize our hands. For many patients, getting cavities in the front teeth can be quite disconcerting. There is always a worry that the fillings will show and be noticeable. This is not always the case. If the cavities are caught early enough, the preparation to the teeth will be minimal, resulting is less loss of tooth structure and filings that are not noticeable. The main reason being that when the cavities are small enough, our approach is from the back, or the palatal side, as is illustrated in the photos. ⁣
⁣
Here our patient had 4 cavities in the front teeth, all of which were restored from the back and all of which are barely perceptible upon restoration. This is the best approach to dental care. Catch problems when they are early. The results will always a conservative treatment with better aesthetic outcomes! ⁣

This was a very simple treatment of composite bonding to even out the worn edges of the front teeth. The bonding was also added to the back of the teeth to replace tooth structure that had been lost from wear. There were chips on the lateral incisors which is not visible in the photos that were also restored. ⁣
⁣
Notice the posture of our patient’s lips after the treatment! This is very often the case. Patients smile bigger after we address an aesthetic problem that has been bothering them for some time. ⁣

A simple procedure that has significant impact for our patients. We are so happy to be back doing what we love to do! ⁣

The return to work has had many challenges. There has been a lot of focus on all the PPE we need. Working with the extra PPE has required a great deal of adaptability but it is doable. ⁣
⁣
The other challenges are also formidable. Social distancing in the office amongst staff and patients has meant that we have had to rework our schedule and adapt to a longer work day. I cannot thank my staff enough for being able to pivot so quickly. ⁣
⁣
Wearing a mask all day long is tiring. What I find most difficult is trying to communicate with my patients with a mask. It is incredible to realize how much we rely on facial expressions (and our smiles) to communicate with others. ⁣
⁣
In the end, we are making it work. Our patients have been fantastic at adapting to the new protocols! Every day that passes seems like we are getting closer to establishing the new normal. ⁣

I finally attached the DSLR back on the microscope today (fully draped of course!) so I hope to have some dental photography posted soon! ⁣

Daily dentistry is never boring dentistry. Some restorations can be quite challenging indeed. This patient presented with a large lesion in the upper right molar but she was asymptomatic and the pulp was vital. ⁣
⁣
In this case, I was fortunate enough to be able to remove the caries without a pulp exposure. The caries pattern was such that I was able to get all the peripheral margins clean and apply pulp protection to the affected dentin closest to the pulp. ⁣
⁣
The composite was layered incrementally, building each cusp incrementally to reduce contraction shrinkage. ⁣
⁣
The patient has been asymptomatic and hopefully will remain so for some time! ⁣
⁣
Materials used: OptiBond FL, Theracal, G-aenial Flow A3, Clearfil Majesty A2⁣
